Being a local community-based NDIS service, Jasmine Care supports people with mental health challenges, disability, or both. Whatever your preferences we will discuss these with you so that we can support you in the best way possible.
This is the normal level of support service, whether that is assisting around the home, allowing someone to help with shopping or helping in getting to appointments, overall continued help to support you to build your skills for those everyday activities.
Assisting with community access whether it is help with shopping or assistance in getting to appointments, overall continued help to support you to fulfill necessary tasks and essential outings. These community access activities will be limited and based on the social distancing and gathering guidance.
We provide the option for you to engage with a support worker by phone or video so that you can still stay connected when you are at home. You can ask your support worker to help with tasks such as the purchase of groceries or picking up medication so that you get everything that is essential for you without having to leave home.
